Members have exceptional benefits included with the health plans we offer. Generally, Medicare and/or Medicaid will cover the cost of home health services for those patients who meet either program’s eligibility requirements. Private insurance companies, managed care organizations, and workers’ compensation plans may also pay for home health services.

Our goal is to return our patients to normal function as quickly and safely as possible. Our rehabilitation professionals work with each patient's physician to develop a personalized care plan. CHRISTUS Trinity Mother Frances Physical Medicine and Rehabilitation offers heated indoor pool therapy, with individualized exercise programs. Our long-term care provides the skilled services of two adult, gerontological nurse practitioners. Dedicated full-time to visiting patients in nursing homes, they make both routine and sick call visits to patients of CHRISTUS Trinity Clinic physicians. Our physicians are specialists in general urologic care and minimally invasive laparoscopic surgical procedures.

Center for Medicare & Medicaid Services regularly gathers feedback information from patients about their experiences with a certain home health agency. In the survey, the patients shares their feelings about services they received, and whether they would recommend the agency to someone else. This information is also very helpful to the agency, telling the agency where to focus their improvement efforts for better health care and services. A general surgeon certified by the American Board of Surgery is one who has training and experience related to diagnosis, preoperative, operative and postoperative management, including the management of complications. Our diagnostics staff is comprised of highly trained professionals eager to identify your specific needs. Once a diagnosis has been made, the journey to recovery continues with your physician as your guide. Other aids and services are available as well, including assistive listening devices such as amplifiers, UbiDuo face-to-face communicator or white boards. Our 24 state-of-the-art surgical suites enable physicians to perform routine and specialized surgeries including expanded cardiovascular, orthopedic, neurosurgical, urological, otolaryngological and endoscopic capabilities. Monday through Friday, patients requiring infusion services can visit the NorthPark Medical Plaza, across from Mother Frances Hospital Tyler. CHRISTUS Trinity Mother Frances Urology Institute and Continence Center is dedicated to providing advanced diagnosis and treatment of urologic conditions.
